چکیده

T he SYMPLICITY HTN-3 (Renal Denervation in Patients With Uncontrolled Hypertension) study was a randomized, sham-controlled clinical study of the blood pressure (BP)-lowering effects of renal sympathetic denervation (RDN), which included ambulatory BPmeasurements (ABPM) as part of the inclusion criteria and as a secondary endpoint. The primary results of the study failed to show a larger reduction in office BP and ABPM compared with those of sham treatment at 6 months (1). In this issue of the Journal, the SYMPLICITY HTN-3 investigators report office systolic BP (SBP) and ABPM after 1 year in most of the patients originally randomized to RDN and in patients in the sham group who did and did not undergo RDN after 6 months (2).

Renal denervation for the treatment of cardiovascular high risk-hypertension or beyond?

Hypertension imposes a major burden of morbidity and mortality and is associated with sympathetic nervous system overactivity. Renal sympathetic denervation has been shown to reduce office blood pressure, ambulatory blood pressure, and sympathetic activity in patients with resistant hypertension. Therefore, the procedure has attracted a lot of attention. Beyond blood pressure, renal denervation...

Device-based approaches for renal nerve ablation for hypertension and beyond

Animal and human studies have demonstrated that chronic activation of renal sympathetic nerves is critical in the pathogenesis and perpetuation of treatment-resistant hypertension. Bilateral renal denervation has emerged as a safe and effective, non-pharmacological treatment for resistant hypertension that involves the selective ablation of efferent and afferent renal nerves to lower blood pres...

اثر سمپاتکتومی عروق کلیوی در بیماران با فشارخون مقاوم به درمان

Background: Sympathetic complex of over-activation kidneys is one of the main causes of primary hypertension (HTN). We aimed to assess the efficacy and safety of sympathectomy using 5Fr mariner catheter ablation on patients with refractory hypertension. Methods: In this prospective cohort study, patients who received three or more anti-hypertensive medications with 160mmHg systolic blood pressur...

Drug-resistant Hypertension: Is Renal Sympathetic Denervation the Answer?

Introduction: Sympathetic overactivity is thought to be a major contributor to the pathogenesis and progression of human hypertension [1, 2]. In particular, renal sympathetic activation results in renal vasoconstriction, increased renin secretion, and enhanced sodium and water reabsorption, all of which contribute to the development of hypertension [3]. Despite this mechanistic understanding, a...

Sympathetic Renal Denervation for the Treatment of Hypertension: A Review of Current Progress, Limitations, Techniques, and the Future

Although the etiology of hypertension is complicated and multifactorial, the role of sympathetic overactivity as part of the pathogenesis in essential hypertension has been well described in the literature. A promising new treatment modality of Renal Sympathetic Denervation has become prominent with very enthusiastic support. This paper suggests that while the support may be overhyped, this new...
